Health Facility Registry (HFR) is a comprehensive repository of all health facilities in India, across various systems of medicine such as modern medicine, Ayurveda, Unani, Siddha, Homeopathy, and others. HFR enables health facilities to get connected to India's digital health ecosystem, providing real-time updated information on health facilities across the country on a national platform. Enrolment in HFR is voluntary, and the process is straightforward and secure. The entire process takes 20-30 minutes, provided all necessary information is readily available with the applicant.
By registering your health facility on HFR, you can gain access to a host of digital services and be a part of India's digital health ecosystem. The aim of HFR is to enable facilities to provide real-time updated information of health facilities across the country on a national platform.
(NOTE: You need to have a valid Healthcare Professional ID to start the registration process. If you haven’t created one, create one as outlined in the steps here.) Here is a step-by-step guide to register your facility on HFR as a healthcare professional/administrator
Step 1: Visit the HPR website & Login with your HPID/Mobile Number
- Visit the official HPR website at https://hpr.abdm.gov.in/
- Choose whether you are a Healthcare Professional or an Administrator/Facility Manager on the login homepage.
Login via HPID:
- Select Login via HPID/Username
- Enter your HPID (<id>@hpr.abdm) & Year of Birth in the highlighted text box.
- Enter the answer captcha. Click on the ‘Submit’ button below.
- Choose to Login via Aadhar OTP OR Mobile OTP OR Password.
- Enter the OTP / Password on the next screen. Click on the ‘Login’ button below.
Login via Mobile Number:
- Select Login via Mobile.
- Enter your 10-digit Mobile Number in the highlighted text box.
- Enter the answer captcha. Click on the ‘Submit’ button below.
- Enter the Mobile OTP. Click on the ‘Submit’ button below.
- Select your linked HPID and Click on the ‘Submit’ button below.
Visit HFR website to initiate registration:
- Click on the “Please complete your registration in HFR here” link.
- Upon clicking “here”, you will be redirected to the HFR registration screen
Step 2: Search & Register Facility
- Select “Facility Manager” as the applicable role.
- Under My Dashboard screen, click on the “Search and Register Facility” button on the top right corner.
- Select “Register New Facility” option. Click on the “Proceed” button below.
- After the prompt to register new facility appears, Click on the “OK” button.
Step 3: Complete the Detailed Facility Information Form
Check Verification of Mobile Number & Email ID:
Submit Basic Facility Details:
- Input “Facility Name”, “Country”, “State/UT” fields.
- Input “District”, “Sub-District”, “Address Line1”, “Pin Code”, & “Geographic Location” fields.
- Input additional non-mandatory fields, as applicable.
Upload Proof of Facility Documents:
- Choose & upload the Facility Building Photograph and Facility Board Photograph. (NOTE: Please choose a clear picture of the facility building and facility board displaying the facility’s name. Maximum size allowed is 5 MB)
- Choose the Address Proof Type & upload the Address Proof. (NOTE: Only .jpg/.jpeg/.png/.pdf filetypes allowed. Maximum size allowed is 5 MB)
- Click on “Add Additional Address Proof” if you wish to add additional documents (not more than 3).
Submit Linked Program IDs (if applicable):
- Check whether NHRR ID is “Verified”.
- Input the remaining details, as applicable.
Submit Additional Facility Details:
- Input “Days of Operation”, “Shift1”, “Shift2”, “About Us” fields, as applicable.
- Select “Facility Operational Status”, & choose the relevant option.
- Click on the “Save & Next” button below.
Submit Facility Type & Ownership Details:
You will be directed to the next input screen which would look like this:
- Choose the relevant option for Facility Ownership.
- Choose the relevant option for Facility Ownership Subtype.
- Select “Profit” as applicable for Private OR Public-Private Partnership Facility Ownership.
- Select all options (Modern Medicine, Dentistry etc.) that apply under “System of Medicine”
- Select the “Facility Type” & “Facility Sub Type”.
- Select all options (OPD, IPD, Daycare) that apply under “Type of Service”.
- Click on the “Save and Next” button below.
- Select “General OPD Services”, for the facilities having OPD Type of Service.
- Select all other options (OPD, IPD, Daycare) that apply under “Services/Specialization” (NOTE: It is mandatory to select at least one specialization)
- For other “System of Medicine” (Dentistry, Physiotherapy, Ayurveda, Unani etc.), select all services/specialization checkboxes that are applicable. (NOTE: It is mandatory to select at least one specialization for each selected System of Medicine)
- Input the count for the different types of IPD Beds (except ICU with Oxygen, ICU beds with ventilator etc.) as applicable and available in the facility. (NOTE: “Total Number of Beds” would be the sum of all types of beds inputted.)
- Input the count for “Total Number of Ventilators”. (NOTE: If left blank, this would be assumed to be 0.)
- Input “Number of Dental Chairs” (applicable if the facility has Dentistry System of Medicine).
- Submit other General Information.
- If the facility has an Imaging Center, input the count of equipment for each imaging service.
- Click on the “Save and Next” button below.
- Read & Tick the check-box for the undertaking terms and conditions.
- Click on the “Submit” button below. (NOTE: You can also choose to check the Registration form to check whether all details have been submitted correctly by clicking on the “Preview” button.
- Click on the “OK” buttons for the pop ups that follow after submission.
Step 4: View Facility Details & Verify the Completion of the Registration Process
On submission of the registration form, a unique facility ID will be generated. All facility information is considered self-declared on submission, and the information of the facility is then verified by respective state/union territory government officials.
- Under My Dashboard, verify that you can find your submitted application form.
- Add Healthcare Professional, as applicable.
- You will receive an SMS & email from ABDM-NHA on your registered Mobile Number & email-id, stating that your details have been successfully submitted.
